Who We Are
The South Australian Aboriginal Chronic Disease Consortium is a collaborative partnership formed to govern and lead the effective implementation of the three plans.
The Consortium has five active working groups including an Executive Group, an Aboriginal Community Reference Group (CRG) and three condition-specific leadership groups representing diabetes, cancer and heart and stroke. The people and organisations involved in these groups are our members.
The Consortium Executive Group
The Consortium Executive Group members are the CEOs from each SA Health Local Health Networks, the Aboriginal Health Council of South Australia, the two Primary Health Networks in SA, as well as the co-chairs of the Community Reference Group and the condition-specific Leadership Groups. The group reports directly to the SA Academic Health Science and Translation Centre and the SA Aboriginal Health Partnership.

The Community Reference Group
The Community Reference Group has cross representation from many South Australian Aboriginal communities and are a critical voice in the Consortium. The role of the Community Reference Group is to provide guidance and advice on the Aboriginal Chronic Disease Consortium; to provide high level cultural and community knowledge across the Consortium, review the progress of implementation of key actions and ensure accountability to the community.

Condition-specific Leadership Groups
There are three condition-specific Leadership Groups – one for diabetes, cancer, and heart and stroke. Experts, clinicians, service providers, non-government organisations, advocates and community members participate on the Leadership Groups.

Coordinating Centre and Secretariat
The Consortium Coordinating Centre and Secretariat provides operational and executive support to the South Australian Aboriginal Chronic Disease Consortium.

Members of the Consortium
Collaborating partner organisations will provide in-kind and/or financial contributions to support the implementation of the plans through the Consortium. Activities may include service reorientation, workforce training, policy improvements, sector collaboration and coordination and the development of new services where required in a culturally appropriate manner.
Other organisations that get involved with the Consortium will be included as new members.

Friends of the Consortium
The “Friends of the Consortium” are any individuals or organisations who have an interest to support the vision and goals of the SA Aboriginal Chronic Disease Consortium. Friends can join at any time.
Given the holistic view of Aboriginal health and the importance of other sectors to improve health the Consortium will welcome “Friends” from outside of the health sector.
Friends will receive regular updates on the Consortium’s activities as well as invitations to get involved in specific issues.
The Consortium Coordinating Centre has been meeting with organisations to inform them of the Consortium priorities and identify any opportunities to influence and support their work as it relates to Aboriginal people.
